Ready for Resolution Questionnaire

l. I have already or am willing to make a complete break with a gay identity, lifestyle, people and experiences.

3. I am a saved/born-again/committed/and active Christian, and, have already or willing to tell my pastor or someone in a responsible position in church about my struggle and desire for resolution? Weigh these two together for a rating.

4. I am in a good Biblical church which is supportive of counseling efforts such as this one; and I have a personal devotional life with Jesus Christ, believing God is working to heal me? Together

5. My overall motivation for resolution comes from myself vs. from others or social pressures? It's based on moral, spiritual and biblical convictions. I don't want to remain conflicted about homosexuality any longer?

I have had some experience with the opposite sex which was positive rather than a great disappointment (when younger or recently), and also, I do not feel an intense dis-attraction, antagonism or outright repulsion towards the opposite gender? I can even be attracted to the opposite sex at times. Weigh these together for an overall rating.

7. I am within the optimal age for working at resolution; (ideal but somewhat relative) , that is, roughly between 25-40?

8. I have been free of early negative sexual experiences (especially molestation/abuse), and, my sexual attitudes/knowledge and view of sexuality are not severely distorted in my estimation? Weigh these two together for a rating.

9. I believe that I have an overall psycho-social maturity (career/job satisfaction, self-image/ life-adjustment, success in other areas of life, etc.) which I consider adequate and functional.

10. I have been relatively free from debilitating mental health/emotional problems or other mental disabilities; suicidal attempts history? I do not have a history of psychiatric breakdown/hospitalization? Weigh these together for a rating.
